📄 Fraud Reporting Documentation Toolkit
This toolkit provides ready-to-use templates and forms for reporting individuals, organizations, or groups who promote, sell, or manufacture fraudulent products or services within the Targeted Individual (TI) community.
These resources are designed to empower victims and concerned citizens to take action through legitimate legal and regulatory channels.
🧾 1. Complaint Form – Federal Trade Commission (FTC)
Purpose: To report deceptive marketing, unsubstantiated claims, or scam sales.
Instructions:
- Visit: https://reportfraud.ftc.gov/
- Select: “Report Now”
- Category: Choose “Products and Services” or “Imposter Scams”
- Include evidence such as:
- Screenshots of promotional claims
- Scan reports or device specs
- Communications with the seller
Template Language:
I am reporting an individual/group promoting fraudulent technology marketed to victims of targeting or directed energy harm. The product claims are not supported by scientific evidence or validated testing. Supporting evidence is attached.
🧾 2. FCC Harmful Device Filing
Purpose: To report RF-based devices being marketed or used illegally (e.g. jammers, unlicensed RF scanners, fake detectors).
Instructions:
- Visit: https://www.fcc.gov/general/form-2000e-interference
- Choose: “Interference” or “Unauthorized Equipment”
- Include:
- Device make/model
- Operating frequency (if known)
- Claims made by the seller
- How it misleads consumers
Template Language:
This complaint concerns an RF-emitting device being marketed to vulnerable individuals as a diagnostic or protective tool. Based on technical review, the device does not meet FCC certification standards and is being misused for fraudulent gain.
🧾 3. State Attorney General Consumer Complaint
Purpose: To initiate legal review and possible investigation at the state level.
Instructions:
- Google: “[Your State] Attorney General Consumer Complaint Form”
- Fill out the form online or download PDF version
- Include all receipts, communications, and a clear timeline
Template Language:
I am reporting deceptive business practices involving medical or electronic diagnostic claims targeting vulnerable individuals. The product was not medically approved, and its technical specifications are provably incapable of what is claimed.
🧾 4. Evidence Dossier Template (For Court or Media Submission)
Purpose: To organize your evidence into a single, easy-to-read file for legal or journalistic review.
Contents:
- Cover Page (Title, Date, Target of Report)
- Timeline of Events
- Technical Review or Expert Analysis
- Screenshots, Emails, Scan Reports
- Witness Statements (if applicable)
